The DOT service animal form, explained
When people say "the service dog form," they mean the U.S. DOT Service Animal Air Transportation Form. It's the only service-animal form a U.S. airline can require for the cabin. If any flight on your trip is scheduled for 8 hours or more, the airline can also ask for the DOT Service Animal Relief Attestation.
What's on it
The form asks about your dog's health, training, and behavior in public. You check the boxes and sign it yourself. Nobody else, including PetPassGo, can attest for you.
The 48-hour rule, without the myth
It isn't "always 48 hours before." The rule works like this:
- If you booked more than 48 hours before your first departure, the airline may ask for the form up to 48 hours in advance.
- If you booked closer to departure than that, the airline can't require it ahead of time, but it can ask you to fill it out and hand it over at the departure gate.
- When an airline asks for it in advance, it has to let you send it electronically or on paper.
Check your airline's page for how it wants the form. Your Travel Pass links straight to it after you Plan my trip and pay.
One task, not four
Download the form, fill it in after you book, send it the way your airline asks, and you're done. Your Travel Pass keeps it as one checklist item, with a reminder if the airline wants it early.
No letter replaces it
No internet letter, registry, or certificate stands in for the DOT form. Use the official one; it's free. A psychiatric service dog does not need a PSD letter.
